If a person is praying three or four rakats, then he should stand up after tashahud saying "Allahu Akbar" and raising his hands as he did in Takbir Tahrimah. Start his recitation with "Bismillah...and then Surah Fatihah." In the third or fourth rakat of fard prayer recitation of Fatihah is sufficient. There is no need to say another Surah. But a person praying sunnat or nafl prayer can read a Surah after Fatihah. After recitation he should continue to complete his third rakat (or fourth rakat if he is praying four).
After the completion of last rakat he should sit for tashahud as described above (as he sat after praying 2 rakats).
After Tashahud he should read Salat Alan-Nabi (Darud) as follows:-
"Allah humma sal-li 'ala Muhammadin
wa 'ala ali Muhammadin,
Kama sal-layta 'aala Ibraheema
Wa 'ala aali Ibraheema
innaka hameedum majeed." (contd)
"O Allah! send grace and honor on Muhammad(saw) and on the family and true followers of Muhammad(saw) just as you sent Grace and Honor on Ibrahim(as) and on the family and his true followers. Surely, you are praiseworthy, the Great."
…"Allah humma barik 'ala Muhammadin
wa 'ala ali Muhammadin, Kama barakta 'ala Ibraheema
Wa 'aala aali Ibraheema
Innaka hameedum majeed. "
"Oh Allah, send your blessing on Muhammad (saw) and the true followers of Muhammad (saw) just as you sent blessings on Ibrahim (as) and his true followers. Surely you are praiseworthy, the Great."
